Social Work Coordinator (Rural Homeless Coordinator)

The incumbent is responsible for providing case management for a caseload of veterans in the HUD-VASH program. The incumbent is responsible for assisting the veteran to identify and pursue recovery goals leading to the retention of housing with the HUD-VASH voucher. This may include, but is not limited to, assistance with identifying and pursuing community resources, VA health and mental health care, substance use resources, assistance with budgeting and increase in income, and other goals as identified by the veteran. The person does manage a HUD-VASH case load, but this is a Rural Coordinator. They will work in rural communities to coordinate homeless Veterans Services. As part of their responsibilities they will:
Performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes. Verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation. Responsible for collecting and submitting HUD-VASH Program participant data into the HOMES system as outlined by NEPEC evaluation procedures. Responsible for providing HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment to house and maintain veterans in HUD approved stable housing. Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manager. Participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH Follows local policy and SOP for the Medical Center and the HUD-VASH Program. This includes following policies and documentation requirements as required for CARF and JC accreditation. Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Responsible for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. Works with VA Medical Center multi-disciplinary teams and community agencies Participate in a rotation of on-call social workers as needed, including modification of tour of duty to accommodate patient care needs on weekends. Provides a full range of social work services which includes case work, individual, family and group counseling, discharge planning, case management, advance directives and other services as appropriate. Assigned to the HUD-VASH Program and works in coordination with the HUD-VASH supervisor and reports to the Deputy Chief of Homeless Programs. Provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. Establishes procedures and coordinates the screening and admission process for the Homeless program by developing and maintaining criteria that recognizes program eligibility, prioritization for program acceptance, and requirements of the PHAs. Transport veterans in government vehicles in support of meeting the goals identified in the veteran's treatment plan. Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Participates in the orientation/teaching of graduate social work students, students of other disciplines, and medical center personnel on the bio-psychosocial aspects of homelessness. Travels throughout the Southern Colorado area as part of assigned duties. Responsible for providing outreach based case management for homeless veterans in Southern Colorado, primarily the San Luis Valley. Work Schedule:

Not required This position requires the incumbent to operate a government motor vehicle, you must submit with your application a copy of your current, unrestricted state issued drivers license. Failure to do so will result in being eliminated from consideration. If you are found to have a restricted license it will be grounds for termination. Basic Requirements:
Citizenship. United States Citizen Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. GS-12 Social Worker Program Coordinators. Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty, i.e., Suicide Prevention. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget. Individuals assigned as social worker program coordinator must be licensed at the advanced practice level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-master's degree training for a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs shown below. Demonstrated Knowledge, Skills, and Abilities 1. Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. 2. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. 3. Ability to supervise multidisciplinary staff assigned to the program. 4. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s 5. Ability to provide training, orientation, and guidance within clinical practice.
